It is part of our policy in UTMD INTEGRATED SERVICES LIMITED to keep all employees informed of the need for safety consciousness in all we do. We all know that unwanted accidents are; they cause death, pain and suffering to the victims and their families. They waste labour, money, and time. They damage plants and equipments, disorganise arrangements and generally reduce productivity. The total loss due to accidents is enormous therefore the prevention is vital.

This manual will serve for easy reference to the company’s health, safety environmental and welfare procedures and guidelines.

UTMD INTEGRATED SERVICES LIMITED considers the oil industry as very vital and of strategic importance to our economy. Bearing this in mind, to be able to contribute as well as stay in business, we must dream, think and act safely always. This message has been planted permanently in the brain of our workmen without any doubt, guessing or half measures. They have been indoctrinated to ask questions when in doubt as precision is the key word in Oil industry.

We ensure that our employee believe in the sanctity of human lives and the environment we operate. The first major responsibility of each and every one of our employee is to ensure that we maintain accident free profile in our assignment. Therefore, as an important task, that we must always comply with all applicable international, federal, state and that of our multinational client safety laws. This is of course, in addition to all recognized Industrial practices worldwide.

The above measures notwithstanding, it is the responsibility of our management and workmen to prevent accident at all cost. Consequently, it is mandatory to wear good personal protective equipment. Our workers have been instructed to always abide with all the safety rules.

We are being able to achieve accident free environment in our assignments due to the high level of understanding we share with Loss Prevention Department regarding safety.


Generally our safety policy will be geared towards the followings:-

  • Eliminate all forms of hazard.
  • Properly maintain all plants and equipment.
  • Provide the site with sign towards indicating work in progress, restricted areas, maximum speed allowed, the way to follow etc.
  • Maintain flagman/men to assure observance of the signboards.
  • Investigate any accident to reveal all elements responsible in order to remove them immediately.
  • Recognize excellence in safety efforts of the Organization.
  • Organize regular safety training programmes to instruct workers on how to write safety reports.
  • Accidents are not acceptable.
  • All injuries and occupational illnesses are preventable.
  • Health, Safety & Environment (HSE) is everyone’s responsibility.
  • Line management is directly responsible for preventing injuries, occupational illnesses, and protection of the environment.
  • HSE consideration is a condition for mobilization of personnel, equipment & materials.
  • Training is an essential element for effective implementation of this policy.
  • HSE audits shall be conducted on a regular basis.
  • Safe, healthy and environment-friendly work practices shall be enforced and anything to the contrary corrected promptly.
  • It is essential to investigate all incidents/accidents to document same and communicate findings to the entire workforce.
  • HSE performance forms part and parcel of the assessment & remuneration of every staff.
  • Preventing injuries, occupational illnesses, and environmental degradation is good business and profitable.
  • People are the most critical element in the success of the HSE program.





We consider the safety and health of our employees and protection of our facilities as paramount. Consequently, the following safety procedures have been designed and applied.


The real danger on site lies on negligence on the part of workers. We shall emphasize this to them, there is no accident in Machines rather it is caused by wrong operator. There must be vigilance at all times if we must have accident-free report. The most important aspect is before embarking on any assignment, a suitable work permit must be obtained. The work permits must be in accordance with the lay down procedure set out by the company.

Adequate preparation must be made as regards equipment for our maintenance work on site, in case of work hazards. We have realized that accident occur through ignorance or carelessness. In case of our workmen meeting any serious hazards they should draw inspiration from Loss Prevention. For instance, if we handle maintenance job that involves draining, make sure that there is no oil spill and fire extinguisher must be kept nearer to the worksite.


Fire is among the most dreaded hazards especially in the Oil and Gas industries. An average job site presents a very wide range of inflammable materials, ranging from paper, sawdust, wood, diesel, petrol, to oxygen and hydrogen gases. Because of the highly inflammable nature of most of these materials, industrial fire most of the time is difficult to control and costing so much on human and material losses.

Generally the best approach to this danger is prevention, however, the following safety precautions must be followed:-

  • Every worker or employee should be familiar with the fire emergency procedure of the site, location or work place.
  • Know where the nearest extinguisher is installed and know how to operate and use it.
  • Obey ‘No Smoking’ signs, smoke only in designated places and use an ashtray.
  • Extinguishers must be checked at regular intervals, at least once a year.
  • Remember to refill any extinguisher used as a cause of fire or leakage.
  • Know the different types of extinguishers variable and the types of fire they are meant for.
  • Never store inflammable products in confined areas.
  • All welding jobs at the site must be covered by work permit before operation.
  • Avoid welding on combustible walls or floors.
  • Never use flammable liquids as cleaning fluids.
  • Do not overload electrical sockets.
  • Do not temper with fire equipment and raise alarm if you discover fire.
  • Don’t get trapped while fighting fire and be sure to abandon the premises without hesitation to pick up your belongings in case of fire.


  • Arrange file drawers so that the lower drawers bear the heaviest load.
  • Never open two file drawers at a time and always close file drawers and desk drawers when not in use or when you leave the area.
  • Avoid standing on make-shit devices or swivel chair while reaching high places.
  • Use sharp office equipment with care, don’t place them pointing upwards on your tables or in your pocket.
  • Avoid throwing sharp objects or other breakable materials into a waste paper basket.
  • Waste paper baskets should not be used as ashtrays.
  • Avoid heater and other warm object to contact with furniture to prevent fire hazard.
  • Doors should be opened carefully to avoid injuries to other users and never stand in front of a door that opens towards you.
  • Loose carpeting should be tack down immediately.
  • Any broken items or glasses or desktops should be discarded immediately.
  • Use moisteners for envelops to avoid mouth cuts from paper due to licking of envelops.
  • Do not overload electrical sockets.


Road accidents cause the highest proportion of deaths in Nigeria as a whole. Adequate precautions are therefore required by every road user (motorists, cyclist and pedestrian) to avoid mishap on the road.

Safety Procedures:

  • Before driving a company vehicle, a driving permit apart from the statutory license must be obtained.
  • The Porto clinic and Porto Glare test must be taken to assess reaction time, depth perception, colour vision glare recovery etc. The result helps the driver to identify his limitations and to make adjustments.
  • Company’s internal proficiency test should be conducted before being deployed to drive any of the company’s vehicles.
  • Attend defensive driving training and practice the techniques.
  • Drivers and front seat passengers must wear their seat belts while in motion.
  • No passengers or worker shall be conveyed in an open vehicle and pick-up trucks.
  • All vehicles shall be equipped with portable fire extinguishers, two triangular reflectors, one jack, wheel spanners and a spare wheel etc.
  • The necessary safety check should be routinely carried out daily such as oil, water, brake fluid, lights trafficators, horns, wipers etc.
  • All drivers must observe speed limits, stop at all cross road and avoid overloading.
  • No employee will be permitted to drive under the influence of alcohol or drugs or without a valid and appropriate driving license.
  • Getting on or off a vehicle while it is in motions is strictly forbidding.
  • Smoking near a vehicle being refueled is strictly prohibited and do not refuel a vehicle while the engine is running.
  • Take brake when driving long distances because a tired or sleepy driver is a potential danger to himself and other road users.


Drowning is a thing to consider in water borne operations or transport, by and large, most safety precautions observed by road transport users are also applicable here. In additions every employee involved in any water borne operations is required to be a swimmer and those who are not yet swimmers are advised to join swimming training programmes.

The following regulations should also be observed:-

  • The Quarter-master is in charge of the craft and all instructions and safety regulations passed by him must be complied with.
  • Always remain seated in the cabin at all times unless instructed otherwise.
  • Embark and disembark only when the craft has been fully secured and long sided.
  • The transportation of workers in water borne and swampy areas shall be by house-boats and speed-boats and not dugout boats.
  • It is forbidden to smoke during water transportation because of the danger of fire hazard.
  • Anyone seeing a worker falling over-board should sound an alarm and throw the nearest life belt in his direction and place himself so that he cannot lose sight of the man. The alarm should be “Man Over-Board”.
  • When alarm is sounded for abandoning the vessel and the order to prepare for evacuation has been given, everyone onboard must abide to the safety procedures as follows:
    • Put on life jackets and vests;
    • Loosen your ties, laces and belts;
    • Follow the order procedure to evacuate when given;
    • Proceed to the evacuation area without luggage or baggage.
    • When jumping into water for safety and survival purposes, please abide to these safety procedures:
    • Your life jacket must be firm on you;
    • Choose the safety area to jump as to avoid falling on objects or other dangerous materials.
    • Make yourself lighter by removing your boots and heavy wares before jumping.
    • Place one hand over the mouth and nose and firmly grasp the life jacket over either shoulder with the other hand.
    • Keep your elbows downwards tightly against the chest and take a deep breath before jumping into the water.
    • Jump with feet first, keeping your body erect and legs together.
    • Upon surfacing, make every effort to stay together with others and assist for the survival of others if necessary.
    • Necessary first aid treatment should be given to any one in need.


Emergency may come in different forms, and the company needs to be informed by:

  • Stating the nature of the emergency
  • Location
  • Name of the company
  • Name of the person involve.
  • In case of fire, which is common in industry like this, the fire emergency number should be called and information like these should be given.

-Location of accident

-Extent of injury

-Name of Person Reporting

-Client, Contractor or Sub-constructors name.


All employees should familiarize themselves with evacuation of the premises where work is being done. Should an alarm sound, stop work immediately and move to designated assembly area until “All Clear” signal is given.

In case of any serious emergency, which our company is not available to provide immediately, our workers are advised to call or rush any injured person to Clinic.


It is better to ask ten times than to do the wrong. We shall always advice our workers to avoid guess work; they should ask questions were they are in doubt.

  • Obtain proper briefing before any job is embark upon to determine the nature and scope of work, the dangers and hazards involved, the appropriate safety procedures required, the right equipment to be used.
  • Do not attempt to operate any equipment which you are not thoroughly familiar with.
  • No smoking is allowed at work, matches or lighters are prohibited from work places.
  • All equipment must be handled with care. Any danger or fault in any equipment must be reported immediately so that the equipment will be repaired or replaced. Use of faulty equipment is likely to result in accident for such it is not to be encouraged.
  • Peace, harmony, seriousness and cooperation must be maintained by workers at the work place. Fighting, quarreling or dangerous practical jokes are prohibited. Team work must always be encouraged.
  • Sleeping on duty is a serious offence; offender may be terminated since that may lead to negligence of duty.
  • Report an unsafe situation to your manager/supervisor.
  • Contractors must be familiarized with safety procedures before entering any work site.
  • Supervisors should discuss each job with all employees. Job planning contributions is essential since it increases awareness, input and reduces man-power requirement.
  • Drinking intoxicating drinks or taking hard drugs while on duty is strongly prohibited.
  • Loose, dangling type clothing or jewelry should not be work while working in field and plant operations for these are particularly dangerous around rotating equipment.
  • Observe and obey all warning signs.
  • Operator of machine/equipment must be informed of the capacities and the must not exceed the stated capacity rating.
  • Do not at any time use any apparatus of your own design or improvise safety equipment of any type without the permission of your safety supervisor.



The purpose of this procedure is to define the responsibilities of all personnel to ensure that all incidents/accidents are noted, reported, recorded, investigated and the results fed back to the relevant parties in order to prevent a reoccurrence.


  • An incident: An unplanned, unwanted event which has the potential to or may have caused injury to people, or damage to equipment, material, company reputation or environment.
  • First Aid Case (FAC): Any work-related injury, which is simply treated on site and the injured person returns back to work immediately.
  • Medical Treatment Case (MTC): An injury or occupational illness that requires medical attention off site by a medically qualified person and that does not result in any lost day at work. However reference off-site for precautionary attention like X-ray, laboratory test (which results return negative) are exempted from this definition.
  • Near Miss: Any incident that did not result in any injury to personnel, damage to equipment/environment but had the potential to do these.
  • Lost Time Injury (LTI): Any work-related injury that prevents the personnel form going back to work the next day.
  • Fatality: Any work related injury or illness resulting in the death of an individual irrespective of the time between the two.
  • Recordable Case: Any medical treatment case plus lost time injury plus fatality.
  • Restricted Work Case (RWC): Any work-related injury that result in the injured person returning to his work but not to his normal duties.

It is the responsibility of all personnel to report all incidents, minor or major, immediately. This will enable proper investigation to be carried out and action to prevent reoccurrence of such incidents put in place.


On the occurrence of any incident, either the victim or observer should make reports within one hour to his supervisor. The supervisor then report the incident (with the use of incident report form given in appendix 1) through his superior officer to the Project Manager and also to the HSE Manager. The HSE Manager on getting the report also forwards it to the Project Manager. It is the duty of the Project Manager or his designate to report the incident to the client and their parties, as soon as possible, but in any case, within 24 hour of occurrence.


All incidents should be investigated. The level of the investigation and composition of the investigation team depends on the risk level. Each investigation team should have a presiding officer and a reporting officer. Other members of the team should be chosen based on expertise vis-à-vis the type of incident.

On the occurrence of any incident, the HSE Manager determines the risk potential using the incident potential matrix. The Project Manager should constitute the investigation. If the risk potential is high, the investigation team must be presided over by the Project Manager. For medium risk, the Works Manager or Administrative Manager presides. Low risk incident potential investigation team can be presided over by either the HSE Manager or any other senior site supervisory personnel.

Investigation reports should be accurate and factual. It should include the following facts;

  • Details of victims
  • Summary of events leading to incident
  • Statement of victims and witnesses
  • Pictures/diagrams (if necessary)
  • Summary of findings
  • Actions required in preventing reoccurrence of such incident (stating action parties and latest completion dates.)

A copy of all accident/incident reports should be sent to HSE Manager for inspection.

  • Type of equipment/material involve
  • Name of the person
  • The company he is working for
  • Age of the person
  • Extent of injury
  • Full detail of incident
  • Action taken etc.

All accident should be reported to Loss Prevention Department and Medical Department for First Aid Treatment, delay may result in serious damage. On a major accident the affected person should be rush to Hospital or Clinic (as stated in this manual) to be attended to by Medical Doctor.


We must ensure our staff undergoes Safety Orientation Program of the company before embarking on any serious job. eg.

  • The use of hard hat
  • The use of safety boot
  • Raincoat
  • Goggles
  • Ear Muff etc.
  • In addition to this, employee intending to work offshore should also undergo orientation on the followings:-
  • Helicopter Safety Orientation
  • Fire course orientation
  • Medical first aid orientation
  • Road Safety orientation
  • Offshore weekly safety orientation
  • Swimming orientation
  • The use of life-jacket
  • How to use fire extinguisher
  • The use of permit-to-work etc.

At the end of it a certificate will be issued to the participants.


Prior to commencement of work, Site Inspection Team shall inspect the worksite and ascertain whether any health or safety hazards exist. This is to correct the identified hazards before commencement of work or will take steps to prevent personnel exposure to the hazard(s). While on site, Loss Prevention representative have the right to periodically inspect operations and worksite for the purpose of monitoring compliance. Whenever any hazard occur, the possible cause should be investigated and way to eliminate further occurrence should also be carried out.

The safety officer shall from time to time make a worksite safety inspection to find out if workers conform to the safety rules of the company.

He can on his inspections be seen to be keen on safety by insisting on the wearing of safety gear, on keeping gateways clear and unobstructed, on ensuring that strangers at the worksite conform to safety discipline.

He can insist on the proper tools to be used and on defective tools withdrawn.

He can ensure new workers are instilled with his sections keenness for safety right from the start. He can explain why, are not merely tell workers that jobs must be done in a certain way.



The purpose of this procedure is to define the HSE training courses that should be undergone by relevant personnel while working in this project.


The Project Manager has the responsibility to ensure that adequate resources are provided for training programmes. He also has to ensure that all personnel undergo courses for which they have been nominated.

The HSE Manager should do the training needs analysis of all personnel. He is responsible for nominating personnel for relevant training based on the result of the analysis and upon confirmation of the nominees by the Project Manager, for the Organization of the training courses.



Site induction should be carried out for all new arrivals on site. After the client’s induction course, all personnel should pass through the joint enter internal induction before being allowed access into the work site. The safety officer will facilitate this induction course with the various senior supervisors as resource persons. The subjects to be covered in this course will, in the main, consist of:

  • HSE policy and HSE management structure
  • HSE/Site Organization
  • Government regulations/client requirements
  • HSE targets for the project
  • Policy on toolbox meetings
  • Incident/accident reporting procedure
  • HSE regulations
  • Accident prevention
  • Emergency response procedure
  • Fire Prevention/protection/fighting
  • First aid and life saving
  • Site facilities


In addition to the HSE induction course, further specific training should be given to selected personnel based on training needs analysis done HSE personnel.

The courses will be held at the joint venture training room and client’s training school.

The training courses shall include, but not be limited to the following:

  • Defensive driving courses
  • Basic fire fighting
  • Safe hoisting/lifting operations
  • Basic first aid
  • Batch plant HSE awareness
  • Scaffolding & Ladder safety
  • Waterborne operations
  • Personnel Health & Hygiene
  • Environmental pollution & spill control

At the end of the course certificates of attendance will be issued to the trainees.

Safety training is organised for employees by the management of the company to promote safety and prevent industrial hazards this will enable workers to become conscious of unsafe conditions and acts while on duty.

Safety induction course will include:

Review of safety regulation

Introduction of immediate supervisor

The supervisor shall give him specific instructions regarding the equipment he operates

The hazard involved the precautionary measures required.

The emergency measure which includes the following should be noted:

Fire drills

Location of fire protective equipments for fire fighting and how to use them.

Water survival:

This concerns swimming and use of life jackets for offshore jobs.

Helicopter Safety instruction is always given to anybody prior to boarding helicopter for the first time.

First Aid:

For first aid management, it is the duty of every employee to participate in all First Aid training sessions.

General Training:

Concerned with maintenance jobs, crane and fork lift driving etc., where applicable.


Safety meeting shall be continually held at interval on site. This is to be supervised by supervisors or any of the sectional heads.


HSE meetings shall be a regular feature on site. Several types of meetings shall be convened as the need arises but the regular ones shall be:

Management HSE meetings

Supervisors HSE meetings

Toolbox meetings

General review meetings


This shall be convened t least once a month and presided over by the project manger. Regular attendees shall include the works manager, HSE manager, camp doctor, administrative manager, logistics/plant manager, technical manager, and QA/QC manager. The agenda for the meeting shall include review of the implementation of the company policy, review of the minutes of general meeting (and status of actions arising therefrom) HSE performance score cards of all the sections among other things.


These meetings shall mostly be between the heads of sections and their supervisors and convened on a weekly basis. The agenda shall be group-specific and serve also as a training session. Discussions shall centre on the stage of the work, inherent hazards and measures to avoid accidents from such hazards. Information from the management to the supervisors and vice versa shall be passed on this forum.


Toolbox meetings shall be held on a daily basis before start of work and shall last between 5 to 10 minutes. It shall be convened with each work group and presided over by the supervisor or foreman. The agenda shall be discussion of the day’s work, inherent hazards and prevention of accidents.


This shall be convened on a monthly basis. Attendance shall be mandatory for all staff and agenda shall include the review of performance in the past months, HSE presentation and other matters.


HSE suggestion boxes shall be located at strategic positions on site. This shall enable staff to air their views or make useful suggestions.

Aside from meetings, HSE information shall be communicated to staff via elaborate safety signs, memos/newsletter, performance shall be communicated to client, corporate management and workforce on a weekly basis with the use of the attached format (Appendix 2)


There shall be an award for the best section in terms of HSES every quarter. The mode of selection shall be based on performance indices agreed by the management. Prizes and certificates shall accompany these awards.

The meetings should preferably be held on unit basis to cut down on size and make room for more close discussions.

The purpose of meetings is to identify and solve health problems at work site.

They can also be used to disseminate accident.

Each employee at the meetings should be encouraged to feel free to present positive ideas, which can help the work to go on smoothly.

All appropriate meeting recommendations must be promptly carried out by all members. A report of each safety meeting be prepared and a copy shall be sent to Loss Prevention and to all members.


Safety Awareness Programme is designed to further promote safety at our work locations. This involves the provision of safety signs at strategic places such as:

  • Reduce Noise level in this areas
  • No smoking area
  • Watch your steps, hold hand rails
  • Do not rely on luck, use appropriate protective equipment
  • Identifying and analyzing current safety requirements of on-going jobs and proposed jobs with a view to ensuring the provision of all necessary safety inputs.
  • The workers should be familiarized with the safety procedures and updates where necessary, for instance, in case of accident which may lead to death or serious damage, lies realize the benefit of an insurance cover both for the workers and the company equipment.



The purpose of this procedure is to ensure that all excavation work carried out on the project is planned, controlled and executed in a safe manner in order to prevent accidents and to ensure that excavation construction and use meet the required standards.


This procedure covers the execution of all excavations (manually or mechanically) to be carried out at the work site.


The Works Manager has the responsibility for the management of all excavation activities on the site in line with this procedure.

The competent person who is appointed by the Works Manager shall be responsible for the execution of all excavation work on the site. It is his responsibility to ensure that the excavation work is performed by persons who are fully competent and ensuring that the precautions as stated on the relevant work permit are fully adhered to.


  • Prior to the commencement of excavation work, the Client’s permit shall be obtained.
  • At excavation depth of 1.2m or soil bank greater than 1.5m, suitable shoring shall be installed or the sides sloped back to the safe angle. Shoring systems shall be designed by a competent person; meets accepted engineering standards and shall be made of material of good condition and proper dimension.
  • The use of personal protective equipment during excavation work shall be in accordance with the requirements of the permit to work.
  • A competent person shall carry daily inspection of the excavation work.
  • Personnel shall not be permitted to work underneath loads or in places where they could be struck by any part of a mechanical excavator.
  • Suitable stop blocks shall be used to prevent equipment over running the edge of excavation where vehicles dump materials into the excavated area.
  • Where site personnel, equipment, or members of the public are required or permitted to cross over an excavation, a close planked bridge or walkway with standard guard rails shall be provided and kept clear of excavated materials or other tripping hazards.
  • Safe means of access and egress to and out of excavations shall be provided at intervals of 7.5 meters.
  • Suitable barriers shall be erected where there is a risk of persons or vehicles falling into an excavation. If this risk exists at dark, warning lights shall be used to mark the limits of the work.
  • Excavations shall not be left open longer than necessary.
  • Where there is reason to suspect a potentially hazardous atmosphere, excavations greater than 12 meters shall be tested for hazardous atmosphere or oxygen deficiency.
  • Before any excavation work is done, the company in question must check with the company’s representative regarding the location of all known underground utilities such as electric cables, pipelines, telephone cables and tank etc.
  • Warning signs and barricades such as “Road Closed or Road Diversion”, Man at work etc. must be inserted nearer to the work place to avoid any hazard.
  • Excavation below 1.5m, earth support structure must be provided to avoid caving in. All excavation must conform to applicable government regulations.
  • All excavation must conform to applicable Gas/Oil Industries, State and Federal Government regulations.


It is the duty of the safety representative to ensure that work permit is obtained before the commencement of any job. He is to ensure that all rules and regulations stipulated on the job permit are strictly adhered to. The work permits include the hot work permit, excavation work permit, entry work permit, or cold work permits. These are to be raised by the safety personnel and endorsed by the appropriate personnel. The work permit has to be renewed after lunch break or if job was stopped for any reason for over two (2) hours.



The purpose of this procedure is to give the necessary standards and responsibilities to ensure safe working conditions in the performance of welding and flame cutting operations on the project. The procedure shall apply to all tasks anywhere on the project site.


Hazardous area: An area where the use of machinery and electrical equipment may lead to fire or explosion due to the formation of an explosive atmosphere.

Hot work: Work that implies a risk for fire and explosion.


It shall be the responsibility of the Project Manager through the field superintendents / supervisors to ensure the implementation of these procedures.

It shall be the responsibility of the HSE Manager to monitor compliance with the procedure.


The HSE department and the Client (if desired) shall inspect all welding equipment before being mobilised for work on site. The equipment shall be re-inspected on a monthly basis with the use of a prepared checklist and the result of such inspection documented in a manner suitable for audits.

  • Only well trained and experienced personnel will be allowed to do welding.
  • Before the commencement of any welding operation the supervisor should clarify from the safety personnel whether or not “HOT WORK” permit is required.
  • Welders should be properly attired in appropriate personal protective equipment (PPE) such as fire-resistant aprons, had hats, goggles with suitable filter lenses, hand gloves before the commencement of work. Such clothing should be free from grease, oil and other flammable material.
  • A suitable fire extinguisher should be kept ready for immediate use at places where welding is done and a personnel to act as fire watch (if the situation warrants).
  • At places where welding machines are operated by internal combustion engines, adequate ventilation should be provided.
  • Adequate precautions should be taken to protect persons working or passing near welding or cutting is being done on materials containing toxic or harmful substances or liable to produce toxic or harmful fumes, adequate precautions should be taken to protect workers from such fumes either by exhaust ventilation or respiratory protective equipment.
  • No welding or flame cutting operations should be allowed in hazardous areas without a permit-to-work.
  • When welding and flame-cutting operations are being carried out in confined space adequate ventilation, by means of exhaust fans or forced draught should be constantly provided. In addition no blowpipe should be left unattended inside a tank or vessel other confined space during work interruptions and workers should take all necessary precautions to prevent unburned combustible gas or oxygen from escaping into other confined space. An attendant should also watch the welder from outside.
  • Welding equipment should be switched off when not in use.
  • Electrodes should only be inserted in the holder with insulating means such as insulating hand gloves. Rods should not be left inserted when not in use.
  • Live parts of electrodes holders should be inaccessible when they are not in use and should not be allowed to come into contact with metallic objects.
  • No welding must take place while the welder is standing on a metal ladder or in water.


  • Cylinders for compressed gases should be distinctively identified as to their contents, to ensure the safety of the worker.
  • Cylinders should be adequately protected against excessive variations of temperature.
  • If the cylinders are in storerooms, suitable danger signs should be displayed outside.
  • Storerooms should be adequately ventilated.
  • Smoking will not be allowed in the storeroom.
  • Cylinders should be segregated for storage by type of gas.
  • If necessary to prevent danger, there should be a fire-resisting partition between cylinders of oxygen and acetylene. Or in the alternative, the distance between the storage areas of oxygen and acetylene cylinders should not be less than 6m.
  • Empty cylinders should be kept apart from charged ones.
  • Valve protection caps should always be in place with cylinders are not in use.
  • No tools or other objects should be placed on the top of a gas cylinder.
  • Cylinders should not be knocked, dropped or subjected to sudden shocks.
  • Hammering or other violent means should not be applied to open valves.
  • The valves of the cylinders should be closed immediately after emptying.
  • Oxygen cylinders should not be allowed to come into contact with oily or greasy materials.
  • Suitably designed carts should be used for the transportation of gas cylinders on the construction site.
  • No slings, hooks or magnets should be used to hoist cylinders.
  • When in use, cylinders should be held in an upright position by straps, collars or chains but in such a manner that they can be quickly removed in an emergency.
  • Welders should not tamper with safety devices and valves on gas cylinders.
  • Gas must not be taken from cylinders unless right pressure-reducing regulator has been attached to the valve.
  • Only hose lines specially designed for welding and cutting operations should be used to connect an oxygen-acetylene torch to gas outlets.
  • Hose lines for oxygen and for acetylene should be different colours.
  • Care should be taken that a hose does not kink or become tangled, stepped on, run over or otherwise damaged.
  • When torches are being changed the gases should be shut off at the pressure-reducing regulators and not by crimping the hose.
  • Torches should be lit with friction lighters, stationery pilot flames or other safe source but not with matches.
  • These have a high potential for personnel injuries and fires. Therefore, the following safe work practices shall be adhered to:
  • Obtain a hot work permit before cutting or welding.
  • Inspect work area to ensure that there are not combustible materials.
  • Place fire extinguisher in work areas.
  • Do not cut or weld on any container that may have contained combustible materials.
  • Ground the frames of all welding machines.
  • Remove gauges and torches from cylinders at the end of the work day.
  • Wear face or eyes protection against flying objects.
  • Use only spark ignites for lighting touches not matches cigarette lighter or any form of naked fire.
  • Oxygen and Acetylene valves must be protected with caps.


  • Only experienced personnel may erect scaffolds and all parts are to be inspected against any deformity.
  • Before putting scaffolds into – use ropes, hooks and blacks must be inspected and certified okay.
  • Employees work on any platform or floor more than four feet (4ft.) high must ensure they have protective guardrail intact.
  • Scaffolds should be tested to uphold four times the maximum intended load.
  • Tools, equipment, or any material should not be thrown from one level to another, up or down.
  • Scaffolds should be safe and convenient for the employee at work.
  • Workers on a scaffold shall wear approved safety belts and tail lines securely attached to an independent support on the structure being worked on.
  • Swimming scaffolds shall always be lowered to the ground and lashed to the structure being worked on when work is halted.
  • Where persons are required to work or pass under scaffolds, the scaffolds shall be provided with a screen between the board and the guardrail etc.



The purpose of this procedure is to make all personnel on the work site aware of their responsibilities for ensuring that all their activities, with regards to hygiene and housekeeping are performed with the aim of maintaining at high a standard as is reasonably practicable.


The Project Manager is responsible for the health and welfare of all personnel on the site and shall be responsible for the implementation of this procedure, and for ensuring that hygiene and housekeeping standards are maintained throughout the site by instigating the regular performance of inspections:

All personnel on the site shall ensure they are aware of and abide by the requirements enclosed within this procedure.

The HSE Manager shall be responsible for ensuring that all personnel are maintaining high standards of hygiene and housekeeping and shall monitor this by regular inspections. He shall also be responsible for issuing instructions and training as required.


General Requirements

It is the duty of all personnel on the site to maintain high standards of personal hygiene and housekeeping, and to ensure that all areas of the site are kept in a clean and orderly condition.

Housekeeping means maintaining the necessary standards of cleanliness and tidiness at all workplaces site wide.

The three main reasons to do housekeeping are:

  • To reduce accidents agents and risk of fire.
  • To conserve space, materials and effort.
  • To create safe working conditions.


  • A high standard of personal hygiene should be maintained on the site at all times.
  • Protective clothing must be kept clean and in serviceable condition.
  • Overalls must be kept as clean as possible in a serviceable condition and laundered at regular intervals. Overalls and clothing saturated with oil, grease, chemicals etc. must be changed and laundered.
  • Handling of certain substances, particularly if hydrocarbon based, can cause dermatitis. Barrier creams, or other forms of skin protection, shall be used as necessary. Further, the appropriate clothing should be used.
  • Washing of hands before the consumption of food is essential in all cases, particularly when work has involved the handling of landed products and chemicals etc.


Good housekeeping is essential on the site to the different type of materials and equipment used and stored, which must be immediately accessible. Access routes to safety, fire-fighting and survival equipment must be kept clean and unobstructed.

Paints and flammable materials shall be isolated and stored in specially designated places. Missing or damaged guard rails shall be reported immediately. Oil spills must be cleaned up immediately.

The following is a summary of basic housekeeping precautions.

  • Site access roads are to be kept clear and unobstructed at all times.
  • Adequate signs should be established throughout the site to indicate the various areas i.e. office, laydown areas, workshops, as well as clear project signs at the entrance to the site.
  • Building stairways should be kept clear of all materials and should be adequately lit.
  • Stairways should be kept clean of slippery substances.
  • Safe means of access and engross and escape routes should be kept clear and unobstructed at all times, and pathways for pedestrians should be clearly marked and distinct from vehicular travel routes.
  • Parking facilities should be provided in clearly defined areas.
  • The site should have good drainage and be graded in such a way that water does not pool up during construction.
  • Adequate lighting should be provided for all work areas of the work site.
  • Storage areas should be adequate for all materials and equipment to be store and should be clearly marked. Where necessary they should be securely fenced off for security reasons.
  • Improper stacking and stocking of materials is prohibited.
  • Store materials should not protrude into walkways (especially at eye level) and should be well clear of permanent or temporary structures.
  • Storage and waste collection areas should be conveniently located where the workforce will use them.
  • Books and papers should be kept in proper places and not permitted to lie haphazardly around the offices.
  • Areas around working machines shall be kept clean and from obstructions.
  • A specific area for scrap materials should be designated.
  • Deck/floors should be kept clear of slippery substances (e.g oils etc) which can cause slipping or falling injuries.
  • Tools, etc should not be left on overhead work platforms, where they can fall and strike personnel below.
  • While not in use, all power driven construction equipment should have the ignition locked and keys removed, or made immobile by disconnecting the battery or by any other means.
  • Site electrical power should be de-energised at night, except where it is required for specific lighting facilities or by use by a night watchman.
  • Any excavation or obstruction of any kind likely to create a hazard to vehicular or pedestrian traffic at night will be adequately lit and signs posted.
  • All welfare facilities should be maintained to a high standard of cleanliness and hygiene i.e. canteen, rest areas, toilet washing facilities, first aid station.

Poor housekeeping is the cause of a large number of injuries to personnel.


All waste materials should be disposed of in an approved manner. It is forbidden to throw any article on the ground on the site. All items of waste should be placed in receptacles provided for disposal.

No petroleum products are to be intentionally dumped on the site or in the sea. In the event that petroleum products are accidentally spilt the HSE Manager should be notified immediately.

Keeping the workplace CLEAN and TIDY is called good housekeeping. In fact it is the first approach to safety. Good housekeeping means there is a place for everything and everything is kept in its proper place. It means orderliness, it means good Organization. Good housekeeping ensures that:

  • Good housekeeping is a daily responsibility of all employees and cannot be properly attached by only periodic clean-ups.
  • Good housekeeping means keeping tools, materials equipment, building and properties clean and in order.
  • Tools and materials shall not be scattered about the floor or work location while a job is in progress. After the jobs is completed. All tools and materials shall be returned to their intended storage location. The work area should be left clean.
  • Rubbish, debris, waste and useless material usually constitute accident and fire hazards and should be removed as they accumulate. Rags, tars etc., shall be constantly picked up and placed in proper receptacles, which are provided.
  • Spilled oil, grease, and chemicals should be cleaned up immediately to eliminate the hazard of slipping. Spills to navigable waters shall be reported immediately to your supervisor.
  • Hazards such as nails or sharp edges, around tops of kegs, barrels boxes, cans and other containers shall be alienated.
  • All “NO SMOKING” rules must be observed, cigarettes and matches should not be thrown about any how or into dust bins to avoid fire hazards.
  • Access to first aid boxes and fire extinguishers should be free of all obstructions.
  • Electric cables, wires, cord, leads, ropes etc. should be coiled up and kept away neatly from the work, floor, passages, aisles, gangways etc. to prevent tripping and falling.
  • Waste materials are not lumped together but separated according to their kinds and disposed in a manner that will not harm the environment.
  • All electrical materials and equipment are tuned off at the end of the day’s work.


  • Employees working in field or plant operations including personnel involved in work projects at field locations shall wear hard hats, safety glasses and safety boots, and any other personal protective equipment require at all time.
  • Employees shall be fully and properly clothed while on duty. Shirts without sleeves, such as muscle shirts or see through shirts, are not acceptable. Loose clothing shall not be worn around equipment with exposed moving parts.
  • Under no circumstances shall 100% nylon, polyester, or similar external or undergarment clothing be worn by personnel. This also applies to jackets, clothing made of natural fibers is preferred synthetic fabrics tend to melt when exposed to heat and can magnify burn injuries during high temperature accidents. The higher the synthetic content of fabric, the greater its likelihood of melting when exposed to high temperatures.
  • Proper clothing, shoes and gloves may vary depending on one’s job. If you are in doubt as to the proper clothing to be worn, what personal protective equipment you should use, consult your supervisor.
  • Respirators, hearing protection, hard hats, safety goggles, face shields, rubber gloves, aprons, and safety belts or harnesses are furnished for your protection. If you have need for any of these equipment and do not have it, discuss it with your supervisor.
  • Personal protection equipment shall be properly maintained.
  • Liners may be worn under hard hats only if the hat suspension remains in place. Removal of a hard hat suspension destroys the protection given by the hard hat. Hats with damaged shells or suspensions shall be replaced immediately. Drilling holes, cutting, or painting on the shell of a hard hat is forbidden. Hard hats shall be cleaned according to the manufacturer’s recommended methods.
  • Where there is risk of injury from hairs contacting moving machinery parts, combustibles, or toxic chemicals, employees shall confine their hair to a hair net or cut their hair.
  • Before starting any job, evaluate your personal protective equipment needs. The general parts of the body to be protected include:

*Eye and Face

*Hands and Arms

*Feet and Legs


*Respiratory System



Gloves protect the hands. However, exercise extreme caution if it is necessary to wear them around moving machinery.

Clothing that becomes saturated with oil or other chemical substances shall be changed as quickly as possible. Employees should immediately wash any such substance off the skin.

Read warnings on chemical containers and in material safety data sheets, so you will know what you are handling. Take additional precautions as specified by the chemical manufacturer and your supervisor.



It is the purpose of this plan to define:

The procedure to minimise or eliminate the discharge of waste materials in order to avoid negative impact on the environment and

The responsibilities and obligations of the UTMD INTEGRATED SERVICES LIMITED lower tier sub-contractors and employees to ensure conformance to the waste management requirements of this project.


It is the responsibility of the Project Manager and line management to ensure the implementation of this plan.

It is the responsibility of all employees and lower tier sub-contractors to dispose of wastes properly in conformance with the requirements of this plan.

It is the responsibility of the HSE Manager and Environmental Officers to conduct inspections of the job site and camps to ensure compliance and report back to the Project Manager.

Line Management, HSE staff and employees have an obligation to co-operate with Contractor Environmental Manager (and his staff) and external auditors in granting access to documents at the site.


The following are the board categories of waste envisaged to be generated on this project and their management procedures.


Green marked plastic containers (with lids) shall be provided for daily collection of food waste. These containers shall be emptied every 24 hours and waste sent for incineration. Empty food cans and metallic beverage containers shall be segregated and treated as metal waste. They shall be shipped out of Bonny Island and either re-cycled or disposed-off with the Federal Ministry of Environment guidelines. Appropriate records shall be kept.


Sewage from the life camp shall be pumped to the JV camp sewage treatment plant.

Vacuum trucks shall periodically be used to remove sewage from the workers camp to the plant. Tracking records shall be kept.


Medical waste such as expired drugs will be returned to the factory from where they are ordered. Soiled dressings, tissues and cotton wool shall be incinerated on site while other medical wastes like needles, syringes, vials and surgical blades shall be segregated in yellow coded containers and disposed-off in line with the Federal Ministry of Environment guidelines. Appropriate tracking documents shall be kept.


Utilization of papers shall be minimised as much as possible by re-using “used papers”. Waste papers shall be collected in plastic bins and incinerated on site.

Used computer toners and printer cartridges shall be re-cycled. Appropriate records shall be kept.


All empty plastic and metal containers shall be disposed-off in line with the guidelines of the Material Safety Data Sheet (MSDS) of the contents and/or of the Federal Ministry of Environment. Recycling of chemical containers in this domestic circuit is to be avoided.


These shall be collected daily at close of work to enhance good housekeeping. They shall be collected in marked containers and periodically packed an shipped to Port Harcourt for re-cycling. This shall be done in line with the Federal Ministry of Environment guidelines and tracking documents shall be kept.


These shall be disposed-off in line with the MSDS guidelines.


This is envisaged to constitute a sizeable amount of waste at the carpentry workshop. We intend to maximise the use of used wood for temporary facilities and formwork. The surplus shall be given to the local communities as building materials to enhance good public relation. The unusable lot shall be incinerated.


Waste oil shall be collected in marked containers according to type and shipped to Port Harcourt for disposal through reputable waste management contractors. Same for oil and fuel filters. Appropriate tracking documents shall be kept.


These are not envisaged to constitute a sizeable quantity of waste to be generated at site but they shall be collected and appropriately shipped to Port Harcourt for re-cycling. Appropriate records shall be kept.


Concrete waste shall be used for road paving preparation. The surplus shall be given to the host communities for public relation enhancement. Appropriate records shall be kept.



This procedure defines the measures to be taken to ensure that all lifting operations that performed in the course of execution of the project, including the certification and safe use of lifting equipment and crane operations, and performed in accordance with statutory legislation and safe working practice.


Lifting and rigging equipment for purpose of this procedure is defined as:

Slings, shackles, chain blocks, jacks, eyebolts, wire rope, fiber rope, containers and equipment baskets and any other equipment used in the lifting of loads.

A rigger is the person responsible for the control of the lifting and positioning of any loads to be lifted.


It is the responsibility of the Project Manager to ensure the safe use, handling and storage of all lifting and rigging equipment on the project and for the maintenance and certification of the various cranes. It is also his responsibility to ensure that all lifting equipment used on the site are certified and carry the current Contractor colour code. He will delegate the responsibility to supervise and control all lifting operations to the Plant/Logistics Manager.

The Plant/Logistics Manager shall:

  • Check new equipment for conformance with request for (R/S) and purchase order (PO) (types/capacity).
  • Arrange for third party surveyor inspection and certification of all lifting equipment.
  • Ensure that the Contractor current colour code is marked on the equipment.
  • Log a file each certification.
  • Update lifting equipment register.
  • Regularly inspect loose gear for wear and remove service from defective one
  • Ensure that equipment is submitted for re-inspection and re-certification every six months.
  • Ensure that the equipment without valid certification is removed from service and unfit ones destroyed to prevent accidental use.
  • The crane operator shall have the responsibility for the safe operation of the cranes. They should be aware and understand all the applicable safety precautions in crane operations.
  • Rigging/logistics supervisor is responsible for the control of the lifting and positioning of the loads in a safe manner. He shall control the riggers assigned to the lifting operations. He is responsible for the safe loading an offloading of barges at the quayside.
  • Mechanical superintendent is responsible for the planned maintenance of all lifting equipment.


Certification of Lifting Equipment

All lifting equipment in use on the project shall be tested and certified by an independent third party inspection company. The test certificate shall have a validity period not exceeding twelve months and only lifting equipment with a current test examination certificate shall be allowed to be mobilised to site.

Colour Code

The Contractor colour code system shall be adopted for all lifting equipment. The colour will indicate the equipment current certification status and has a maximum validity of three months.


A register will be maintained on site with details of all lifting equipment, date of inspection, name of supervisor and current status.



  • Every lifting appliance operator shall be a competent craftsman, having statutory license and certified by the company safety-training department.
  • No person under 18 years of age shall be in control of any lifting appliance or give signals to the operator.
  • The operation of lifting appliances shall be governed by signals.
  • The lifting appliance’s operator’s attention must not be distracted while he is working.
  • No crane, hoist, winch or other lifting appliance must be loaded beyond the safe working load.
  • During hoisting operations effective precautions must be taken to prevent any person from standing or passing under the load.
  • Operators shall not leave lifting appliances unattended with power on or with a load suspended.
  • No person should ever ride on a suspended load or any lifting appliance not authorised by the competent authority for the conveyance of persons.
  • Every part of a load in course of being hoisted or lowered should be adequately suspended and supported so as to prevent danger.
  • Every receptacle used for hoisting bricks, tiles, slates or other material should be so enclosed as to prevent the fall of any of the material.
  • Materials should not be raised, lowered or swung in such a way as to cause a sudden jerk.
  • The hoisting of loads at points where there is a regular flow of traffic should be carried out in an enclosure space, or if this is impracticable (e.g. in the case of bulky objects), measures should be taken to hold up or divert traffic for the time being.
  • Adequate steps should be taken to prevent a load in course of being hoisted or lowered from coming into contact with any objects in such a manner that par of the load or object may become displaced.
  • Tag lines should be provided and used for guiding when raising or lowering heavy or wide or long loads.
  • All cranes and other lifting appliances must be marked clearly with their maximum safe working load. (SWL).


  • The operator is responsible for ensuring that the movement and position of the crane is both safe and suitable.
  • A 600-mm wide clearance between mobile crane and any fixture must be maintained; where this is not practical, barriers should enclose any place where a man might be trapped.
  • Particular care should be taken when working near overhead power cable such that the boom or jib does not touch live power cable – a “flashover” can occur over some distance, depending on voltage. The minimum recommended safe working distance is the length of the longest boom or jib, plus 6 meters measured with the boom or jib pointing towards the cable, even when the crane will be working in the opposite direction.
  • The stability and load-bearing capacity of the ground where a crane is working must be sufficient to carry the most severe static and dynamic loads, taking into account such factors as the swinging torque’s, wind stresses and shock loading, as well as the weight of the crane and its load.
  • Adverse weathers such as rain and strong wind can rapidly affect the stability of a crane and no crane should be used without these factors first being checked.
  • Outriggers must be used for all lifting operations. All the outriggers must be extended fully and equally on both sides and if the ground is any way suspect, suitable woo pads must be used under each outrigger to reduce the ground area loading.
  • Automatic safe load indicators, radius load indicators and motion limit switches must be lifted to cranes and are essential to the safe operation of the equipment.
  • If an operator cannot see his load during the whole operation, he must have one of more trained riggers/signal men, to enable him to handle the load safely.


  • All chains, slings, hooks, shackles, eyebolts etc. must be clearly marked and the safe working load specified.
  • They must be properly constructed and maintained, free of any defect or damage likely to affect its strength, regularly examined and securely attached to the load.
  • All chains ropes shall be checked on deficiencies at three months interval and a competent person must examine other loose items of lifting gear every six months and a register kept of these.
  • Ropes chains etc. must never be overloaded.
  • All hooks used for lifting must be fitted with a safety latch or should be mousses or so shaped as to prevent the sling eye or load coming off the hook.
  • Slings must be attached to the lifting appliance correctly by an approved method, either by securing the ring directly onto the hook if size permits or by use of a suitable shackle, fitted with the crown of the shackle on the hook and the pin fully engaged.
  • A ring must contact multiple slings or shackle and the load properly distributed so that no leg is overloaded.
  • Any chain sling which has been altered or repaired by welding must be examined and retest by a competent surveyor-engineer before being put to use.
  • Where necessary to prevent bends in slings, corners or load should be adequately padded.
  • Overloading, eccentric shackle loads and misuse can distort shackles: they should be checked regularly for shape and wear.


  • Tower cranes should be operated in accordance with the manufacturer’s instructions.
  • At the close of work, the manufacturer’s instructions for the safety of the crane against wind pressure should be observed.
  • Tower cranes should not be operated in high winds or violent storms.


  • When lifting objects with jacks, the jacks should be set on solid footings, centered properly for the lifts and so placed that they can be operated without obstruction.


  • All personnel working at height above 2m without fully erected and inspected scaffold should be provided with a wear full body safety harness. The harness will be provided with synthetic rope lanyards, which have a minimum length of 1.6m. The lanyard must be connected to a suitable anchorage.

A register of the harnesses must be kept. This register should contain date of last inspection, identification code and location of each harness. Inspection of the harness should be conducted on a quarterly basis and after every stress.
